install.exe also not working. Must've been corrupted in dl. I will try to dl again

Yep. need to install in Japanese locale. Using AppLocale in the install will not work.
just applocale the newly created executable if you need to revert back to your preferred locale.
Installing the english patch still needs your pc to be in japanese locale. After patching, use applocale in the executable if you need to return to your original locale.

Whenever I try the original version of the game with a japanese locale emulator it works until I either try to open the game settings or until I'm at the first stage where I click the first heart icon.
With the english patched version I just get a blackscreen, also with japanese locale emulator
Sounds like something is interfering with the game.
The ' animations ' are just plain mpg files and codec packs can be a problem playing the game properly.
Before clicking the heart button, is the shown screen just a still image, or does the breathing animation play?
There was ages ago similar reports with this engine, the problem was out of date intel shared graphics driver and shitty avg / avira / panda anti virus interfering with the game.
If you can't solve your pc trouble, i suggest to use either a clean windows in hyper-v or wsl with Locale Emulator (japanese locale).
WSL is pretty fast in starting up, but itself doesn't save any settings and you should use a hotlink path on your current hdd to store the games.
To play japanese games, you have to install locale emulator every wsl bootup, but it's a small price if your original windows is busted.
If you want to give WSL a try, save the following as ' Sandbox.wsg ' and alter the path. Running this file will start WSL if it's enabled / installed in windows.

The behaviour is that it's stuck with a still image and the music still playing.
I use Windows Defender and disabling it doesn't work either.
Isn't WSL for Linux executables?
Your movie playback is broken due to unknown reason inside the game, that's 100% the behavior for not playing the files.
Windows Defender was never a known problem.
You can start windows as a sandbox with my file (text example) i've added. Yeah, cut the L, WS aka Windows Sandbox.
How to enable Windows Sandbox on Windows 10
Open Start on Windows 10.
Search for Turn Windows features on or off and click the top result to open the experience.
Check the Windows Sandbox option. Enable Windows Sandbox on Windows 10 (at least version 1903).
Click the OK button.
Click the Restart now button.
After starting the game using local emulator, it should work and look like this:
